LectureVid 1.2: Root Cause Analysis

In this lecture, we shall learn what is root cause analysis (RCA) and how to apply them. Basically RCA is a tool for continuous improvement.  RCA could be performed using a variety of tools. The two commonly used tools discussed in this lecture are:

• Ishikawa’s fish bone diagram
• 5 “Why”s approach

For Ishikawa's fish bone diagram, there are a number of thematic grouping of causes that could be used. We shall discuss 3 of the common methods that are relevant in clinical incidents:
